Sex With Low Back Pain: Which Positions Help, According to Spine Research

Which sex positions are easier with low back pain? What spine research found, how to know your pain pattern and when to see a doctor.

In summary

  • First, find your pattern: does your pain get worse when you bend forward, or when you arch backwards?
  • If bending forward hurts: positions from behind, such as doggy style or spooning, moving from the hips, usually load the spine less.
  • If arching hurts: the missionary position with a pillow under the lower back tends to keep the spine more neutral.
  • Move from the hips, not the lower back, and stop any position that brings back your usual pain.
  • Some symptoms need urgent medical care: see the last section.

Low back pain does not have to mean the end of your sex life. But there is no single "best position" for everyone: what helps depends on which movement triggers your pain. Spine research from the University of Waterloo found that spooning, long recommended to almost every patient, is not the right choice for every type of back pain.

02

Step one: which movements make your pain worse?

Physical therapists often group back pain by the movement that provokes it:

  • Flexion-sensitive: pain increases when you bend forward, touch your toes or sit for a long time.
  • Extension-sensitive: pain increases when you arch your back, stand for a long time or lie on your stomach.
  • Movement-sensitive: almost any movement hurts. Here, stable, well-supported positions and a slower rhythm help most.

If you are not sure which group you are in, a physiotherapist can usually help you find out in a single visit.

03

What the research found

In 2014, Natalie Sidorkewicz and Stuart McGill used motion-capture cameras to record spine movement in 10 couples during five common sex positions. With the results they drew up a guide that matches positions and ways of moving to the type of back pain.

If bending forward makes your pain worse

  • For the person moving behind: doggy style was preferable to spooning, using a hip hinge (the movement comes from the hips while the back stays in one piece) instead of thrusting with the lower back.
  • For the person receiving: spooning or doggy style, supporting the upper body on the hands rather than on the elbows.

If arching your back makes it worse

  • For the person receiving: missionary instead of spooning, with a small pillow or rolled towel under the lower back to keep the spine closer to neutral.
  • For the other partner, the study found that spine movement varied a lot from person to person: choose the position in which you can keep your back still and let the hips do the work.
04

Practical tips

  • Support: pillows under the lower back, hips or knees reduce the effort of holding a position.
  • Let the pain-free partner move more, and slow the rhythm down.
  • Choose your moment: many people have less pain after a short walk or a warm shower than first thing in the morning.
  • Have alternatives: touch, oral sex or mutual masturbation are also part of sex and may be easier on a bad day.
  • Talk about it beforehand: agreeing on a word or signal to stop or change position takes pressure off both of you.

What these studies can and cannot tell you

The Waterloo studies are small (10 healthy couples), only looked at vaginal intercourse between a man and a woman, and measured spine movement, not pain itself. They are a useful starting point, not a prescription. A 2024 clinical framework for physiotherapists estimates that sexual difficulties affect between 37 % and 81 % of people with low back pain, and recommends adapting positions to each person's pattern alongside usual treatment. If your pain persists, treating the back itself is what helps most.

When to see a doctor

See a doctor or physiotherapist if your back pain does not improve after a few weeks or stops you from doing your usual activities. Seek emergency care if back pain comes with numbness or weakness in both legs, loss of feeling around the genitals or anus, changes in bladder or bowel control, or new changes in genital sensation, erections or the ability to reach orgasm. If pain appears in the genitals or pelvis during sex, read our guide to pain during sex.

Frequently asked questions

Is spooning always the best position for back pain?+

No. Spine research found that spooning suits some people but not others. If arching your back hurts, missionary with a pillow under the lower back may be better; if bending forward hurts, positions from behind with hip movement are usually easier.

Can sex make my back pain worse?+

It can if a position repeats the movement that triggers your pain. Most people can keep an active sex life by choosing positions that avoid that movement, using pillows and slowing down.

When is back pain during sex an emergency?+

When it comes with numbness or weakness in both legs, loss of feeling around the genitals or anus, changes in bladder or bowel control, or new changes in genital sensation, erections or orgasm. Seek emergency care.
